1) Make a
new file 100x100.
2) Add a new layer.(It is better to create each buttons in new layer.Then
you can edit each button with out affecting the whole image.)
3) Select elliptical tool.
4) Make a circle using fixed size, for example 20x20 px.
5) Select rectangular tool. Hold down the shift key and make a rectangle
at the middle of the circular selection.(Fig1)
4) Select elliptical tool with fixed size again. Hold down the shift key
and click any where in the image to a circle . Then without releasing
Shift key, press space bar and move the circle to the end of the square.(Fig2)
5) Once you are satisfied with the selection, fill the selection with
desired color.
6) Add layer effect Bevel and Emboss.(Adjust the structure & shading)
7) Select the type tool and type the text.
If you want
you can apply effects to the text layer.

1) Make a
new file 100x100.
2) Turn on the Rulers & Guides. (Choose View > Show Rulers, &
View > Show > Guides).
3) Drag a vertical and horizontal guide as shown in fig.
4) Select elliptical tool. In the tools palette select style as fixed
size with 20x20 px. Make a circle and place that as shown in fig3.

Drag another vertical guide as shown in fig.
6) Duplicate the layer containing the circle. Select the the Move tool and
drag the circle to the far right; until its right edge snaps to the right-most
guide. Hold down the Shift while dragging to make your movement horizontal.

Drag another new guide to intersect with the bottom of the circle.
7) Select the Rectangular Marquee and draw a selection starting from the
top-center of the left circle to the botton-center of the right circle.

Create a new layer and fill with the color..
9) Select the Rectangular Marquee and draw a selection starting from the
top-center of the left circle to the botton-center of the right circle.
10)
Now Link the layer containing circles and square and merge that layers.
Layer> MergeLinked.
11) Now you can add layer effect (Bevel and emboss) to that layer.
